The findings section of each DJS Quality Improvement Report contains a detailed description of the operational strengths and needs associated with each Best Practice Standard. A rigorous system of ratings that summarizes the performance levels for each standard is also reported. This rating system for evaluating each performance measure consists of four defined categories ranging from superior performance to non performance.
| Superior Performance |
|
Strong evidence that all areas of practice consistently exceed the standard across the facility/program; innovative facility-wide approach is incorporated sufficiently so that it has become routine, accepted practice.

| Satisfactory Performance |
|
Performance measure is consistently met across the facility/program; any gaps are temporary and/or isolated and minor; documentation organized and readily available.

| Partial Performance |
|
Expected level of performance is observed but not facility-wide or on a consistent basis; implementation is approaching routine levels but frequent gaps remain; facility had difficulty producing documentation in some areas.
|
| Nonperformance |
|
Little or no evidence of adequate implementation of the performance measure; the required activity or standard is not performed at all or there are frequent and significant exceptions to adequate practice; documentation could not be produced to substantiate practice.
